Antibiotics, blood pressure drugs can be risky mix

Older adults on blood pressure drugs runescape gold known as calcium channel blockers could suffer dangerous drops in blood pressure if they are given certain antibiotics, according to a study out Monday.

Researchers found that patients on calcium channel blockers who were given the antibiotics erythromycin (brands like E-Mycin) or clarithromycin (Biaxin) were at increased risk of being hospitalized for dangerously low blood pressure.

A third antibiotic in the same class -- azithromycin (Zithromax) -- was not linked to the risk. And the researchers say it should be chosen over the other two antibiotics for people on calcium channel blockers.

The three bacteria-killing drugs are what are known as macrolide antibiotics -- the most widely prescribed class of antibiotics worldwide. In the U.S. alone, more than 66 million s were filled in 2008.

For their part, calcium channel blockers are among the most commonly used long-term medications. They include drugs like amlodipine (Norvasc), felodipine (Plendil), nifedipine (Procardia, Adalat) and diltiazem (Cardizem, Dilacor, Tiazac).

"It's not far-fetched that a person would be on a calcium channel blocker and macrolide at the same time," said Dr. David M. Juurlink, the senior researcher on the new study, which appears in the Canadian Medical Association Journal.

The problem with erythromycin and clarithromycin is that they inhibit an enzyme vital to metabolizing calcium channel blockers, explained Juurlink, of the Institute for Clinical Evaluative Sciences in Toronto.

As a result, blood levels of the calcium channel blocker spike, leading to sometimes runescape money severe drops in blood pressure.

Azithromycin, in contrast, does not block that same enzyme.

Researchers have known about these drug actions, but until now there had been no large study looking at the risk of severe low blood pressure among patients on calcium channel blockers and macrolides.

For their study, Juurlink and his colleagues looked at medical records for more than 999,000 Ontario residents age 66 and up who were on a calcium channel blocker at some point between 1994 and 2009.

During that time, 7,100 were hospitalized for severe drops in blood pressure. Of those patients, 131 had been on a macrolide antibiotic in the week before their hospitalization.

Juurlink's team found that erythromycin treatment was linked to a six-fold increase in the risk of hospitalization for low blood pressure. Clarithromycin was tied to a nearly four-fold increase.

Because of the study design, Juurlink said he could not give a precise figure for the absolute risk of severe hypotension from the drug combination. But he said it was "probably runescape accounts very low" for any particular person.

Still, Juurlink added, it appears to be an avoidable risk.

"Doctors should be aware of the risk, and know that there is an alternative out there," he said. "If you have to prescribe a macrolide antibiotic to someone on a calcium channel blocker, it makes sense to give azithromycin."

As for other classes of blood pressure drugs -- like ACE inhibitors and beta-blockers -- they do not seem to be susceptible to the effects of the two antibiotics, according to Juurlink.

The current study focused on older adults because they are most vulnerable to the consequences of drops in blood pressure. They could, for instance, get dizzy enough to fall and break a hip. Or in severe cases, low blood pressure can deprive the organs of blood and oxygen, leading to shock and potentially fatal organ damage.

"Younger patients are better able to tolerate a drop in blood pressure," Juurlink said

However, he added, azithromycin would still be the wise choice for younger people on calcium channel blockers as well.

Juurlink also pointed out that erythromycin and clarithromycin can boost blood levels of cholesterol-lowering statins. In that case, he said, the concern would be increasing the risk of serious muscle damage, a known side effect of statins.

So it would be wise for people on statins to take azithromycin over the other two macrolides, Juurlink said.

"Heart Specialist" film never rises above mediocrity

"There's just got to be an easier way to get a standing ovation," the actor said after receiving a rousing response when he walked on stage to present the night's final award, for best dramatic film.
"It's been a quiet year. Catherine is doing a Broadway show and six months of cancer," the silver-haired actor had joked earlier as he walked the red carpet with his wife, Catherine Zeta-Jones.
Douglas, who announced last week that his malignant tumor is gone, looked fit as he arrived for his first major public appearance since completing treatment.
"After you go through this, you have a greater appreciation of your fans. I feel good. I'm pacing myself and I'm enjoying it. I feel very happy to be here," he said, turning serious.
As those fans shouted his name from the bleachers, he turned to wave and smile.
Brad Pitt hung back, letting his partner, Angelina Jolie, work the media gauntlet at this year's Golden Globes.

At first, Mary Lamantia couldn't quite place the teen who reached out to her just before the holidays, but when he declared on a Facebook page that her son Ryan was his hero, she was determined to remember.
Walter Wetzel had met Ryan nearly eight years ago in a hospital waiting room. Both were very sick — Ryan with brain cancer, Walter with leukemia. Walter, then 9, began making silly faces at the little boy sitting across from him.
Soon after, Ryan, who was 3 at the time, made his way into Walter's chemo room and chatted about going home to change into his Teenage Mutant Ninja Turtles costume and ride his Big Wheel to his cousin Catlyn's house.
It was the start of a friendship that would blossom during other hospital encounters over the next few months, but they would lose touch after Ryan transferred hospitals. Lamantia's memory of Walter faded during the years that followed, a time of needles, MRIs, chemo and radiation.
When she read Walter's Facebook post in December describing Ryan as his inspiration, she was overcome with emotion.
"It took me about half an hour, but right before I was going to bed, I said, 'I remember who he is now,'" said Lamantia, 37, of Lake in the Hills. "Ryan … just walked into Walter's room and started talking to him."
Though they saw each other only a handful of times after that, Walter never forgot Ryan.
"He inspired me to survive my cancer," said Walter, now 17, in remission and runescape gold quite the football player and snowboarder. "Seeing him happy all the time made me happy. How could I be upset if he had it so much worse than me?"
For the past few years, Walter, who lives with his family in Elgin, wondered what had happened to his little buddy.

Without so much as a last name, Walter asked the hospital staff to track down a number or address, but privacy laws prohibited the staff from giving out information.

Ryan had died on Sept. 8, 2005. He was 6.
"I just bawled and bawled and bawled," she said.
When she picked up Walter from school the next day, she broke it to him.
"It hit me pretty hard," Walter said. "I was runescape money crying for a week straight."
The Facebook page was for the Ryan Lamantia Foundation, a nonprofit that Ryan's family formed after his death. The organization raises money for pediatric brain tumor research and holds an annual toy drive near Ryan's birthday to restock his beloved playroom at Children's Memorial Hospital.
Walter logged on right away and left this message:
"I have wanted nothing more than to talk to (Ryan's) parents and tell them (their) son is my hero. … My trips to (the hospital) were always dreadful, until the day I (met) Ryan."

"I don't even think there's words to explain the feelings we have that our son made such an impression on somebody," Lamantia said. "It made us so proud."
After such an immense loss, Walter's words were the best gift she and her husband, Todd Lamantia, could have hoped to receive.
"We always knew (Ryan was special), but to hear it from somebody else, it really runescape accounts means the world to us," said Todd Lamantia, 39, a coffee roaster in Huntley.
Walter wasn't the only person who recalled Ryan's perpetual effervescence.
"He was a little superhero," said Wendy Stellpflug, a nurse at Children's Memorial Hospital and the brain tumor coordinator.
Ryan's initial diagnosis in February 2003 was almost cruel. The doctors told his family he had a "sugarcoating" of cancer throughout his brain and down his spine. The first round of chemo landed him in the intensive care unit, nearly killing him.
At the hospital, Ryan's parents held his hand and tried to ease the pain. The playroom became his haven. He carried Buzz Lightyear around with him, spoke incessantly of the band of crime-fighting turtles and channeled Spider-Man every way he could.
Ryan went through a period when he didn't want anyone, family or friends, to see him lying in the hospital bed, drains coming out of his head. But soon enough, the toddler who just wanted to be a little boy found a way to make it through. He kept his spirits high, even after he suffered two strokes, hearing loss and endured 14 surgeries.
"I can close my eyes and see him coming down the hall, shooting his spider web at me," said Dr. Stewart Goldman, medical director of the hospital's neuro-oncology department, who treated Ryan.
Goldman, who has been caring for kids with brain tumors for 20 years, regards Ryan and his family as examples of courage, compassion and kindness. Even with the outward signs of a tumor — his limp, paresis in his arm — Ryan didn't let his illness stop him.
"He always had a smile on his face," Goldman said.

Last week, they met for the first time since Ryan's death.

She shared the same gregarious personality, eliciting grins from strangers at the grocery store, using the living room couch as her personal springboard.
Meeting Walter for the first time, Sarah Ryann studied him from afar. After he made a few silly faces at her, she ran as fast as her tiny legs could carry her straight into his lap.

Health benefits of some organic produce overestimated

Scottish singer Gerry Rafferty dies aged 63

He will be best remembered for his hit "Baker Street", a 1978 soft-rock classic with a trademark saxophone solo that made it into the top 10 of the British and US charts.
The song, which featured on his solo album "City to City", is still played regularly on radio stations across the world and was said to be earning the singer thousands of pounds a year right up until his death.
The saxophone solo, recorded by Raphael Ravenscroft, is rumoured to have been originally intended for a guitar -- it was only when the guitarist did not turn up that Ravenscroft stepped in and buy ffxiv gil belted out one of the best known passages in rock music.
Before pursuing a successful solo career, Rafferty was a member of the band Stealers Wheel with whom he recorded "Stuck in the Middle with You" in 1972 as part of their eponymous debut album.

"There was no hope. I would never have left him if there'd been a glimmer of a chance of him recovering," she said, cited in Britain's Guardian newspaper.

Public easily deceived by trans fat food labels

This means if a product has 0.49 grams of trans fat, manufacturers can label its trans fat content as zero, the report said.

Brain Size Tied to Richer Social Lives

Scientists in New England have linked the size of the amygdala, an almond-shaped part of the brain tied to fear and fight-or-flight responses, to the size and complexity of one's social network.
The bigger the amygdala, the larger and more intricate the person's social life seems to be, according to the team at Massachusetts General Hospital.
The findings, published this week in the journal Nature Neuroscience, seem to fall in line with previous research showing that primates tend to be more social if they have larger amygdala, though none of the studies have determined whether a bigger-sized amygdala is the reason for the more extensive social network or the other way around.
"We know that primates who live in larger social groups have a larger amygdala, even when controlling for overall brain size and body size," study lead author Lisa Feldman Barrett, of MGH's Psychiatric Neuroimaging Research Program and a psychology professor at Northwestern University, said in a statement. "We considered a single primate species, humans, and found that the amygdala volume positively correlated with the size and complexity of social networks in adult humans."
The researchers said they studied the rest of the subcortical structures within runescape gold the brain but saw no convincing evidence that a comparable association existed between the size of those areas and the complexity of one's social network.
They also found that the volume of the amygdala, located in the temporal lobe, didn't factor in to other facets of people's social lives, like support from others or satisfaction with personal relationships.
"This link between amygdala size and social network size and complexity was observed for both older and younger individuals and for both men and women," study co-author Dr. Bradford C. Dickerson, who is on staff at the hospital's department of neurology and is a neurology professor at Harvard University, said in a statement. "This link was specific to the amygdala, because social network size and complexity were not associated with the size of other brain structures."
The scientists asked 58 participants aged 19 to 83 to report on the size and intricacy of their social networks in standard questionnaires measuring the number of regular social connections each subject had as well as the number of different groups the contacts belonged to.
The participants then had to undergo MRIs runescape money so the research team could collect data about the various structures in their brain. Among the information they gathered in the brain scans was the volume of each person's amygdala.
Feldman Barrett and her colleagues found that those with larger amygdalas had more extensive social lives, results that are consistent with a theory known as the "social brain hypothesis" -- the notion that human brains, specifically amygdalas, have grown and evolved to manage more complicated, modern-day social situations.
AOL's mental health expert Dr. Daniel Carlat said that while the findings are interesting, too many questions are left unanswered to be able to form solid conclusions.
"This type of study is not all that satisfying scientifically because we don't really understand any mechanism for how larger amygdalas would lead people to have larger social groups," Carlat, a psychiatrist in Massachusetts, told AOL Health. "It could be a conincidence or it could be something more meaningful, but at this point we don't know."
The role the amygdala plays in people's behavior is already murky, so decoding what runescape accounts the apparent link between it and social networks really means is challenging, according to Carlat.

"With a result like this, we can spin all kinds of theories as to why a larger amygdala would lead to larger social networks -- or vice versa," he said. "But since we don't have any clear understanding of what the amygdala does, any of these theories remain speculation."
He said that since that portion of the brain is thought to control fight-or-flight responses, it may make sense that it would be bigger in those with more people around them.
"We believe the amygdala has to do with how we relate to fear. If you have a large network of people around you, there are a lot more opportunities to process the fear response," Carlat said. "If you think of the amygdala as the fear muscle, there are a lot more chances to flex that muscle."
Feldman Barrett said more work has to be done to identify why the trend appeared in her findings.
"Further research is in progress to try to understand more about how the amygdala and other brain regions are involved in social behavior in humans," she said. "We and other researchers are also trying to understand how abnormalities in these brain regions may impair social behavior in neurologic buy ffxiv gil and psychiatric disorders."
