A Bedside Guide to Mechanical Ventilation

A Bedside Guide to Mechanical Ventilation

Learning how to use a mechanical ventilator can be very challenging and frightening for most residents and other health care students. Many books and articles have been published on this subject, but they often leave the reader confused because they are generally written for pulmonary/critical care specialists. However, most patients will need the same basic respiratory support and will have similar complications. In this book we provide background information and outline strategies for use of mechanical ventilation to make this advanced patient support easy to understand and apply. Use this handbook to learn the basics about mechanical ventilators and to enhance your ICU experience.

The traditional houses in the old center of Sanaa show a wealth of ornaments (horizontal friezes), colours and architectural elements.

There houses have 4-6 stories:
The first floor is for cattle and storage
The second floor is for the kitchen and food storage
The third and higher floors have rooms for the women and children
The top floor contains the meeting room for men, called "mafraj", the most beautiful room of the house.
During long sessions men discuss important matters, while smoking water pipe and chew "qat" (drug from leaves)

The windows are a fascinating feature:
The size of the window increases with the floor number,
1. The most elaborate windows are those from the mafraj on the top floor and consist of 3 elements:
vertical ventilationshafs
2. Double stained glass upper windows ("Kamarija") to light the room; the glass is set in a gypsum frame
3. Lower windows with shutters, to look out

Sometimes - in older houses - the windows of the lower stories have alabaster windows, instead of glass.
The prismatic bay window in the middle of the picture is a "mashrabiya", and is made of turned wood grills. The function is twofold:
1. to permit women to look outside without being seen
2. to place earthenware jars with water or wine; the jars are moistened, the wind blows through the grill, evaporates the moisture and thus keeps the water cool (aircontioning "avant la lettre")

The horzontal ornamental friezes accentuate the different floors and come in a great variety of patterns.
The friezes, edges and window frames are whitewashed, thus contrasting with the ocre surfaces of the fired brick walls.
